What do I need to do in order to receive a reprint of my W-2 for the years prior to and including the year 2007?
- To obtain a reprint of your 2007 or 2008 Form W2, please call 1-800-360-1265 or register for electronic delivery by logging on to https://www.getmytaxform.com/lsbs and follow the steps.
- You will need your date of birth and your social security number. There is a $10.00 administrative fee for the 2008 reprint and a $12.00 fee for 2007.
- To obtain a reprint of your Form W2 for 2006 or prior years, please mail your request as follows:
- Include a self addressed stamped envelope
- Include on a spare sheet of paper your name, Social Security number, address for the restaurant(s) where you were employed.
- Include a payment of $12.00, either in cash or money order made payable to LS Management, Inc.
- Mail the above requirements to:
